This wasn?t the only human study to find positive results. A more recent one conducted in 2012 showed that pomegranate extract reduced PSA increases as well.⁵

The goal of prostate cancer therapy is to keep PSA levels as low as possible. And slower increases are a positive sign. But just what is it about pomegranate that makes it so beneficial? Laboratory studies may offer clues.

Pomegranate Offers Anti-Cancer Properties

The best way to attack cancer is through a multi-targeted approach. Cancerous cells work through different mechanisms to grow and spread.

Phase 3 Study Update of Pomalidomide for Heavily Pre-Treated - MPR

Celgene?announced that its Phase 3, multi-center, randomized, open-label study (MM-003) of pomalidomide plus low-dose?dexamethasone or high-dose dexamethasone in patients with relapsed and/or refractory multiple myeloma was reviewed by a data safety monitoring board (DSMB), who determined the trial met the primary endpoint of improvement in progression-free survival (PFS) at the PFS final analysis.

The study compared pomalidomide plus low-dose dexamethasone to high-dose dexamethasone in patients who were relapsed on or refractory to at least two prior therapies that must have included both lenalidomide (Revlimid; Celgene) and bortezomib (Velcade; Millennium). Patients in the pomalidomide arm received 4mg of oral pomalidomide on Days 1?21 of each 28-day cycle, with patients ?75 years also receiving 40mg of oral low-dose dexamethasone and patients >75 years receiving 20mg oral low-dose dexamethasone on Days 1, 8, 15 and 22 of each 28-day cycle, until disease progression. Patients in the comparator arm ?75 years received 40mg oral high-dose dexamethasone on Days 1?4, 9?12 and 17?20 of each 28-day cycle, and patients >75 years received 20mg oral high-dose dexamethasone on Days 1?4, 9?12 and 17?20 of a 28-day cycle, until disease progression.

The key secondary endpoint of the study was overall survival (OS; with alpha control), and other secondary endpoints included safety, response rates and time to progression. At the OS interim analysis, the study crossed the superiority boundary for OS. Improvements in PFS and OS were both highly statistically significant and clinically meaningful. As a result, the DSMB recommended that patients who had not yet progressed in the high-dose dexamethasone arm should be crossed-over to the pomalidomide plus low-dose dexamethasone arm.

Pomalidomide is an IMiDs compound.

New York Times Digital Subscription Growth Doesn't Offset Ad Sales Slump

The New York Times Co.'s digital subscriptions business continues to grow at a steady pace, but it's not enough to offset bigger declines in advertising revenue at The New York Times, The Boston Globe and The International Herald Tribune, the company indicated in its third-quarter earnings statement released Thursday.

Collectively, the three papers added approximately 83,000 digital subscribers in the third quarter, bringing the total number of subscribers to 592,000, an 11% increase from last quarter. The bulk of those new subscriptions -- 57,000 of them -- were purchased from The Times and IHT.

Both print and digital advertising revenues continued to decline at 10.9% and 2.2%, respectively, resulting in an 8.9%, drop in overall ad revenue "largely due to the challenging economic environment, ongoing secular trends and an increasingly complex and fragmented digital advertising marketplace," the Times Co. said. Its real estate classifieds were the hardest hit, dropping nearly 20% year-over-year. Digital advertising now makes up nearly a quarter of total Company advertising revenues, up a few percentage points from the same period a year ago.

Circulation revenues increased 7.4%, thanks to increases in digital subscription sales and print subscription pricing for the Times and the Globe. The Company decided to erect metered paywalls around its news websites in late 2011 in an attempt to offset foreseeable declines in advertising revenues with gains in circulation revenues.

Total revenues declined 0.6% to $449 million year-over-year. Net income was $2.28 million, or 2 cents per share -- an 85.4% drop from the third quarter of 2011, when the Times Co. sold off a number of investments. Operating profit declined 59.6% to $8.51 million year-over-year.

Shares of the Times Co. were trading down 13.5% to $9.20 late Thursday morning.

The Company is expected to welcome new CEO Mark Thompson on Nov. 12, but some -- including New York Times public editor Margaret Sullivan -- are questioning whether Thompson's offer should be revoked because of a sexual abuse scandal that occurred under his watch at the BBC.

Amazon spending leads to quarterly loss

Amazon.com?reported a larger-than-expected quarterly loss on Thursday as it spends heavily to expand existing operations and develop new businesses. Its outlook also fell short of expectations.

Amazon reported a net loss of $274 million in the third quarter, or 60 cents per diluted share, compared with net income of $63 million, or 14 cents a share, in the period last year.

Part of the loss related to an impairment charge from Amazon's investment in daily deal company LivingSocial.

Excluding items, Amazon reported a loss of 23 cents per share, down sharply from a profit of 14 cents a share in the same period a year earlier.

Revenue jumped 27 percent to $13.81 billion from $10.88 billion a year ago.

But the results missed analyst estimates: Wall Street had expected Amazon to report a loss excluding items of 8 cents a share on $13.92 billion in revenue, according to Thomson Reuters consensus estimates.

"The overriding theme, the shift to third party, is making the revenue number less meaningful. I think revenue is not indicative in figuring out the future profitability of the company," said Jason Helfstein, an analyst at Oppenheimer.

For the fourth quarter, Amazon said it expects revenue of $20.25 billion to $20.75 billion, shy of the $22.79 billion analysts currently expect.

"On the surface it looks pretty weak, (and there are) pretty significant FX (foreign exchange) headwinds," said Needham analyst Kerry Rice. "If you add that back on the top line it looks roughly better-than-expected, but much lower earnings on a GAAP basis. I assume it's likely a result of continued investment in fulfillment centers."

The earnings release also hyped Amazon's signature product, the Kindle, and made numerous comparisons to the iPad mini,?Apple's just-released challenger. CEO Jeff Bezos said Amazon's breakeven approach is working, and that the Kindle Fire HD is its best-selling product across the site worldwide, though he gave no specific sales figures.

"Our approach is to work hard to charge less. Sell devices near breakeven and you can pack a lot of sophisticated hardware into a very low price point,? Bezos said in the press release.

Will Google ever lose its crown as the most popular search engine?

Perhaps one of the most lucrative prospects anyone will ever stand to earn ridiculous amounts of cash from is successfully monetizing the most essential wants/needs of our species. Since the birth of the Web, search has become one of those essential wants/needs, and monetizing it has earned companies billions upon billions of dollars. Naturally, the head honcho of search is Google, but how long will it be that way? What would it actually take to dethrone them as the search engine of choice? I see two primary components to that question: 1) what the majority of people want; and 2) what an alternate search engine stands to offer.

Using Bing as an example of a competing search engine, according to recent advertisements of theirs, Microsoft is claiming that people prefer Bing over Google nearly 2 to 1.

As one might expect, there's a choice selection of fine print to follow up with to get to the bottom of that "nearly 2 to 1" conclusion, but every time I see commercials like that, all I imagine are dollar signs in the eyes of the companies running such advertisements. Why? At the end of the day, the only reason Microsoft wants people to switch to Bing is the same reason Google will fight tooth and nail to retain their users: because of how much there is to profit from search. Sure, the Bing team is tasked with building a solid product that they believe in and genuinely feel is ready to be proven better than Google, but what is it really going to take for someone to topple Google?

Realistically, Google is a juggernaut and their services are more than enough for the average searcher (which is, to say, the majority). Because of that, companies like Microsoft have to first convince those people that there's a problem with Google, and then offer their search engine as the solution to that problem. But to do that, they have to appeal to as broad a base as they can. There's no way they could stand to gain more users by saying "well, here, specifically, are all the ways we're better than Google." If they can make everyone take away what's most relevant to them, then that's going to achieve the most satisfactory results.

But as I said, the majority of people are just fine with Google -- much in the sense that the majority of people are just fine with Coca-Cola. I mean, a Pepsi-Cola might come along one day, but the difference is that a Google alternative will likely never be as sweet as the difference between Coke and Pepsi.

Let me step back for a moment just to clarify that, while I thoroughly enjoy Google's services, this post is in no way a brand-loyal offering. I use a multitude of search engines, but only because my search needs extend FAR beyond those of the typical searcher. Instead of looking for movie times, weather, or random info here and there, I'm looking for information residing on FTPs, ranges of sites that are all hosted on the same IP address, unprotected devices connected to the Internet, etc. (And delving back into the soft drink analogy, I happen to prefer Pepsi, but I also enjoy Coke and plenty of other tasty alternatives.)

Let's try this: Take a moment and ask yourself what you want from another search engine that Google doesn't already offer. Then, for all the things you come up with (if any), are you sure Google doesn't already offer them within their vast line of search offerings? Maybe you want better search results, but is it a matter of Google not being good enough, or could you stand to learn just a fraction more about power searching so as to quickly and simply refine your search results?

Whatever the case may be for you, for most people, their search needs are more than covered by Google. And at this point, Google is such a household name with search that the uphill battle for anyone offering an alternative is going to be quite arduous. But the search ecosystem is so much larger than that, too! There are the end users, and then there are the advertisers; and right now, Google is really the only search engine any company cares to market within because that's where all the search traffic is.

So, while Google has the lion's share of searchers and advertisers, and so long as the search engine paradigm exists as it is today (and will in the foreseeable future), Google will remain the top dog. But even more than that, I legitimately don't believe a company will come along and offer an index as extensive as Google's for quite a while -- at least, not enough to convert the majority of average searchers who can already find what they're looking for. And to that end, the greater point I'm trying to make is that it's not even about someone coming along and offering better search results, a better UI, or otherwise; it's about someone coming along and convincing practically everyone that to find the answers they seek, they need to do something other than "Google it."

While that changed regularly, the one thing that never changed was my desire to be a wife and mother.?That wish came true in August, 2008 when my then-boyfriend and I found out we were expecting a baby. We got married that October and settled down to start our family.

Pregnancy wasn't what I had envisioned.

My first trimester was absolutely awful. I had morning sickness that routinely lasted through the afternoon. I was so sick that I had to quit my job sooner than I'd planned. I can't remember when the sciatic nerve pain began, but it was so severe that many days, I could hardly get out of bed; that is, unless I wanted to end up on the floor. I'd anticipated some mood swings but what I experienced was over-the-top severe. The amount of weight I gained was much higher than it "should have been," even though I was watching my diet and exercising.

Childbirth was absolutely awful.

I was pushed to have an induction. After 32 hours of back labor and abdominal contractions, followed by three hours of pushing, my baby was born.

I was exhausted.

But I thought, "It's over now - this is when the fun starts!"

That wasn't quite the case.

My mood swings became steadily worse - I'd find myself in tears for no reason or over stupid things. I'd be sad one minute, filled with rage the next, and numb immediately after that.

I spent a lot of time simply rocking my baby and staring out into space. I can't remember what I thought about during those days, but they weren't happy thoughts. I was constantly exhausted but couldn't sleep at night. It was awful. I had no idea what was wrong with me.

I felt like a horrible wife and mother.

One night, I couldn't sleep. I found myself standing at the top of the stairs looking down. I thought about how miserable I was and how easy it would be to throw myself down.

I shook myself and walked away, deciding maybe I'd better go to bed before I did anything dumb. As I headed to bed, the thought popped into my mind: my husband still had pain medications left from his ankle surgery. It'd be so easy to take a bunch of those before going to sleep.

The intensity of these thoughts rattled me and I realized that what I was experiencing wasn't the normal "baby blues" and that I needed help.

My husband is in the Army and, mostly still asleep, recommended I call the Battalion chaplain first. The Chaplain told me that I needed to go straight to the Emergency Room at the hospital on base. He would meet me there.

I was diagnosed with Postpartum Depression and ended up hospitalized twice. I was on medication as well as seeing a therapist for six months before some lab-work finally showed that I'd developed hypothyroidism (did you know pregnancy can cause hypothyroidism? I didn't!). Treating the hypothyroidism also treated the PPD and I was able to come off of the antidepressants.

Postpartum Depression is a monster and a beast; a nightmare I thought I would never wake from, but I did.

I just wish I hadn't waited so long to ask for help.

This is the Detroit Tigers' time.

The next 10 days begin a window that will extend two, maybe three more seasons. And nothing less than at least one World Series title is going to be acceptable.

Not for 83-year-old owner Mike Ilitch, who desperately wants to add a World Series trophy to four Stanley Cups with the Detroit Red Wings.

Not for a franchise that hasn't won it all since Sparky Anderson's 'Bless You, Boys' in 1984.

And not for the legacies and Hall of Fame candidacies of Jim Leyland, Miguel Cabrera and Justin Verlander.

Taking absolutely nothing away from the San Francisco Giants ? a talented team riding an emotional high, and one that unexpectedly went all the way only two Octobers ago ? it's lined up for the Tigers.

But they must take the final step now, or face mounting pressure to do so before their window of opportunity closes.

"The best manager in baseball,'' is what Tigers' president/general manager Dave Dombrowski said about Leyland amidst the post-ALCS title celebration.

The recent retirements of Bobby Cox, Lou Piniella, Joe Torre and Tony La Russa make the 67-year-old Leyland the game's winningest active manager with 1,676 regular-season victories ? 95 more than Dusty Baker.

But Leyland needs that second World Series title to help legitimize his boss' claim. He's just one of a handful of active managers (Mike Scioscia, Ozzie Guillen, Joe Girardi, Bruce Bochy, Charlie Manuel) with one and Leyland's came long before any of the others, with the 1997 Florida Marlins.

Terry Francona, recently hired in Cleveland, where he won't be winning any more soon, is the only active manager with two World Series titles.

Leyland's other shot came in 2006, when the Tigers failed in the favorite's role ? upset in five games by the St. Louis Cardinals, and beating themselves with pitchers' fielding errors in the process. They'll carry that same burden favored role again.

But this isn't the playoff-inexperienced bunch Leyland took to the 2006 postseason. It had been 19 seasons since the Tigers' last playoff appearance, and just three seasons prior, the 2003 team lost 119 games.

There were no superstars on that roster, either ? just a group of standout veterans such as Magglio Ordonez, Carlos Guillen and a rejuvenated Pudge Rodriguez. Verlander won 17 games as a 23-year-old, but Kenny Rogers was the ace, and 38-year-old closer Todd Jones made too many save opportunities interesting.

In contrast, this Tigers team has become one of the game's priciest at $132-plus million (fifth-highest), is coming off two consecutive division titles, and is in its third postseason in Leyland's seven seasons in Detroit.

The superstars are right in the midst of peak-production seasons, and almost the entire core of position-player regulars and starting rotation members are in the 25-30-year-old range. So if not now, when? Especially when the AL's two traditional financial juggernauts in New York and Boston face uncertain near-futures.

People forget that Cabrera already has a World Series ring ? as a 20-year-old rookie with the 2003 Marlins. But capping a Triple Crown (and most likely, MVP) season with a World Series title would mean something altogether different to his legacy.

Verlander has made his career goal clear: Cooperstown. Cy Young Awards and MVPs help, but nothing carries as much weight as World Series rings.

There has been no more-dominant pitcher this postseason, and as if he needs any more motivation, don't forget that Verlander cost the Tigers home-field advantage in this series with a terrible first inning in the All-Star Game.

Verlander is lined up for Game 1, while Giants' ace Matt Cain was forced into an NLCS Game 7 start, tilting the rotation match-ups Detroit's way.

In addition to Cain, the Giants received tremendous NLCS starts from Ryan Vogelsong and Barry Zito. But nobody can match the numbers Tigers' starters put up through the first two rounds:

• 5-1, 1.02 ERA, .162 opponents batting average, 9.6 strikeouts per nine innings.

Lastly, the Tigers learned the hard way in 2006 that a long layoff after a quick ALCS can lead to problems. This time, Detroit-area weather cooperated enough to allow them to play intrasquad games with their instructional league players. That doesn't come close to matching playoff intensity, of course, but staying sharp and maintaining timing should result.

Libyan women in Rome to look at women?s rights

Rome, 22 October:

?A group of seven Libyan women are in the Italian capital for a week of training alongside Italian women on equal opportunities and the inclusion of women in society.

The programme, which began yesterday, Sunday, and runs till next Sunday, has been organised by Pari e Dispare, an association for gender equality, with the support of the Italian Ministry of Foreign Affairs and Italian state-owned oil and industrial giant, ENI.

The aim is help to track equal opportunities issues, notably the development of women and their participation in the labour market. It has been devised with particular regard to the participants ? mainly entrepreneurs or young women who have recently completed their studies and are now looking to gain work experience.? It is also a dialogue between Italian and Libyan women on their status and role in the two societies.

It is being seen as a valuable chance to investigate civil rights and the inclusion of women in society and the economy.

The programme kicked off today with a session entitled ?Italian women meet Libyan women? at the Italian Foreign Ministry. It was attended by Italy?s Undersecretary of State for Foreign Affairs, Marta Dass?, the Vice President of the Italian Senate of the Republic, Emma Bonino (who is also co-chair of Pari e Dispare) and the Director General for Development Cooperation of the Italian Foreign Ministry, Elisabetta Belloni.

Increased colorectal cancer risk for extended family members

ScienceDaily (Oct. 22, 2012) ? Women under age 50 who have been diagnosed with endometrial cancer, and first, second and third degree relatives of patients with colorectal cancer may have an increased risk of developing colorectal cancer (CRC), according to two separate studies unveiled October 22 at the American College of Gastroenterology's (ACG) 77th Annual Scientific meeting in Las Vegas. Two other CRC-related studies presented at ACG 2012 investigated the impact of gender, race and obesity on the incidence of adenoma and advanced adenoma detection rates -- suggesting that overweight African American and Hispanic men may be at greater risk for precancerous polyps which if not detected early enough could lead to colorectal cancer.

Highlights of Colorectal Cancer Research from the ACG Annual Scientific Meeting

"Risk of Colorectal Cancer after Diagnosis of Endometrial Cancers: A Population-Based Study

"Women diagnosed with endometrial cancer under age 50 had a "marked increased risk" of being diagnosed with colorectal cancer in a historical cohort study by researchers at the University of Manitoba who linked several large longitudinal databases routinely collected in Manitoba, including the Manitoba Cancer Registry and some of Manitoba Health databases. The researchers followed 3,115 women diagnosed with endometrial cancer between 1987 and 2008 and 15,084 age-matched controls up to December 2009.

Women younger than 50 in the cohort at the time of diagnosis of endometrial cancer had an approximately four -fold increased risk of being subsequently diagnosed with colorectal cancer as compared to the age matched women in the general population. The risk was even higher (seven fold higher) for colorectal cancers occurring in the upper part of the colon (right colon). There was no increased risk for colorectal cancer among women diagnosed with endometrial cancer, when they were 50 years old or older.

Endometrial cancer is a cancer that starts in the inner lining of the womb (uterus) called the endometrium. In the U.S., endometrial cancer is the most common cancer found in women's reproductive organs. The chance of a woman having this cancer during her lifetime is about one in 38, according to the American Cancer Society.

"This study suggests there is an increased risk of colorectal cancer after a diagnosis of endometrial cancer among young women," said co-investigator Dr. Harminder Singh. He said, therefore these patients need close follow-up particularly for colorectal cancers occurring in the upper part (right-side) of the colon. "Colorectal cancer screening should start at a younger age in such women."

"Elevated Risk of Colorectal Cancer in Relatives of Patients with Colorectal Cancer: A Population-Based Study in Utah"

In the first population-based assessment of the risk of colorectal cancer in extended family members of patients with CRC, researchers from the Huntsman Cancer Institute in Salt Lake City, UT reported that first, second and third degree relatives of individuals with colorectal cancer had an increased risk of developing CRC themselves -- with the strength of the association based on the degree of kinship, according to lead investigator, Niloy Jewel Samadder, M.D.

The "degree of kinship" describes the proportion of genes shared by two blood relatives. A person's first-degree relative is a parent, sibling, or child. A first degree relative shares about half of their genes with the person. A second degree relative of a person is an uncle, aunt, nephew, niece, grandparent, grandchild or half- sibling. A second degree relative shares about one quarter of their genes with the person; while a third degree relative of a person is a first cousin, great-grandparent or great-grandchild. A third degree relative shares about one eighth of their genes with the person.

"This study has resolved many of the issues confounded in previous studies where reliance on patient recall regarding family history and lack of verification of reported colorectal cancer diagnoses have made it difficult to quantify the risk of CRC in the relatives of patients with colorectal cancer," said Dr. Samadder, noting that the "biggest strength" of the study is its design.

The retrospective case-control study included 126,936 Utah residents between 50 and 80 years old who underwent colonoscopy between February 15, 1995 and January 31, 2009 at Intermountain Healthcare of University of Utah Health System -- with 3,804 of these patients diagnosed with CRC -- and defined as the case population. For each case, 1 randomly selected age-and -sex-matched control was selected from the population who had CRC-free colonoscopy. Researchers confirmed family relationships through the Utah population database and CRC diagnosis through the Utah Cancer Registry. The results showed that first degree relatives of individuals with colorectal cancer had an 80 percent increased risk for CRC; second degree relatives had an increased risk of 30 percent and third degree relatives had an increased risk of 15 percent.

"Our data support the current CRC screening guidelines and raise the question of whether such screening should be extended to first-degree relatives of patients with CRC diagnosed at or above age 60," said Dr. Samadder, adding that the results, "further support a genetic basis of inheritance for CRC over shared environmental factors."

A key message from the study is that "the risk for colorectal cancer doesn't stop at our first-degree relatives," noted Dr. Samadder. He urged patients to be aware of their extended family histories and encouraged physicians to look beyond a patient's parents and grandparents when assessing colorectal cancer risk.

"Impact of BMI and Gender on Advanced Adenoma Detection Rates in Minority Populations" Body mass index (BMI) seems to have a linear association with advanced adenoma detection rates (ADR) in an African American and Hispanic male cohort, where a trend towards higher right-sided advanced adenomas is also seen in this study group, according to researchers from The Brooklyn Hospital Center.

Adenomas are a type of colon polyp that is considered a precursor for invasive colorectal cancer (CRC) which is the third most commonly diagnosed cancer and the second leading cause of cancer death in both men and women in the U.S., according to the American Cancer Society. It is estimated that there will be 143,460 new cases diagnosed in the United States in 2012 and 51,690 deaths due to this disease.

The study included 895 subjects with a mean age of 73 years and a higher proportion of females compared to males. African Americans made up 74 percent of the group and 26 percent were Hispanic. The study subjects were divided into five groups based on BMI, according to co-investigator Shashideep Singhal, M.D., who noted that males showed a progressive linear incremental trend in ADR with an increase in BMI. Total ADR was 14%, 12.9%, 13.9% and 16.3 % in Groups 1-4 with no statistical differences. Group 5 was excluded from analysis due to small sample size. The incidence of right-sided advanced adenomas was also higher with increasing BMI (60%, 60%, 64.3% and 71.4% in Groups 1-4 respectively). However, this trend was not seen in women in this cohort.

"The study shows that the higher the BMI the higher the adenoma detection rate and that African American and Hispanic men in this study group have the greatest risk for pre cancerous polyps which if not detected early could lead to colorectal cancer," said Dr. Singhal." He said that while these findings need to be confirmed with larger studies, they may have useful implications for designing preventive strategies in high-risk populations -- specifically identifying patients who are at high risk and increasing colonoscopy screening intervals in these patients who are at increased risk for CRC.

When detected early, polyps can be removed during a colonoscopy exam, preventing the development of colorectal cancer. This ability to prevent colorectal cancer through polyp removal is the cornerstone of the American College of Gastroenterology's 2009 screening guideline which recommends colonoscopy as a "preferred" colorectal cancer prevention strategy. The ACG also recommends African Americans undergo CRC screening at age 45 due to increased CRC risk factors. A tremendous body of evidence shows that clearing the colon of polyps, including small polyps, significantly reduces colorectal cancer mortality. When detected in its earliest and most treatable stage, the survival rates for colorectal cancer exceed 90 percent.

"Gender Specific Prevalence of Adenomas, Advanced Adenomas and Colorectal Cancer in Patients Undergoing Screening Colonoscopy"

Researchers from The University of Texas Medical Branch collected colonoscopy data retrospectively from a university-based hospital and included all average risk screening colonoscopies performed between 2006 and 2011 to determine and compare the prevalence and numbers needed to screen for adenomas, advanced adenomas and colorectal cancer in different age groups among men and women. Of the 2388 patients included in the study, 51 % were women.

Overall, men in the study group had a significantly higher prevalence of adenomas (32% vs. 23%) and advanced adenomas (8% vs. 5%) compared to women and the prevalence of CRC was higher in men, but did not reach statistical significance, according to co-investigator Dr. Praveen Guturu, MD.

When the data was broken down between different age groups, the prevalence of adenomas in the 50-59 year age group was significantly higher in men compared to women (29% vs. 19%). But there was no statistically significant difference in the prevalence of adenomas and advanced adenomas among men in the 60-69 age group and over 70 group, when compared to women. "Our study suggests that men might develop advanced colon polyps at an earlier age compared to women and this information will help us design appropriate screening and surveillance guidelines in future,'' said Dr Guturu.

With the average foreclosure process taking hundreds of days, a decision to put off the mortgage payment is often less painful that forgoing other necessities.? Not exactly the change that many Coloradoans were seeking four years ago.

Who Cares about the Department of Health Responsibility Deal ...

My instinctive reaction to the Responsibility Deal brought in by Andrew Lansley was?to be against it ? believing it to be window dressing to cover up the fact that the Department of Health had little intention of using legislative and policy muscle to force the private sector to damage health less and promote it more.

I have nothing against collaborations with private sector organisations, those of us who work at a local level know that local authorities and other local statutory bodies cannot legislate. They rely on influence, negotiation and collaboration to achieve change. Structures like Local Strategic Partnerships were examples of attempts to achieve strategic collaborations for the good of communities ? and Health and Wellbeing Boards will have similar ambitions.

I am also not in favour of creating laws to achieve change if this can be achieved through other means. So it is important to judge the Responsibility Deal on its own terms ? on what it has achieved.

Three ways to assess the Responsibility Deal.

• Strategic ? focussed on priority areas
• System level impact
• Accountable to the public

Strategic Focus

The Responsibility Deal focusses on the following areas.

Alcohol ? primarily about information and also trying to increase the availablity of lower alochol drinks.

Food ? primarily about information, reducing salt, reducing transfats, availablity of lower calorie and healthier food.

Health at Work ? primarily about supporting those with chronic conditions to stay in employment, quality of occupational health services, accountability for sickness levels, food for staff, stop smoking services, staff health checks, mental heath workplace adjustments

Physical Activity ? enabling phsyical activity in communities, promoting active travel, increasing physical activity in the workplace, tackling barriers to participation by the most inactive.

These areas bear little relation to the more coherent model proposed by Duncan Selbie Head of Public Health England which I referenced in an earlier post.

As the above table shows the Responsibility Deal is focussed almost exclusively on Health Behaviours ? aside from a small amount of activity on workplace health, it largely ignores the areas that have the greatest impact on health.

The most recent update (July 2012) tells us that there are 390 partners ? I reckon this probably now stands at about 450. This means that the responsibility deal has been gathering partners at a rate of approximately 6 a week ? I don?t know if this is good or not.

The update also notes that 75% are private sector organisations. So just under 300 are from the private sector.

According to the governments own statistics there are 4.5m enterprises who employ over 23m people; of these 99.2% are small enterprises employing less than 49 employees. 6,320 employ more than 250 employees; 30,475 employ between 50 ? 249.

It is clear that the Responsibility Deal has barely scratched the surface in terms of connecting with the private sector.

Where it does better is when it is working in environments that are dominated by very few companies ? such as food retail where the ?big 4? control about 75% of the market.

I have already indicated that it is very hard to understand the degree of impact because the way in which the information is presented means that it is not possible to get a feeling for the system level impact of the various pledges.

Outputs

The Responsibility deal has got some traction ? for example the Food and Drink Federation Annual Review for 2011 which was published in April 2012 devotes a page to the deal and they have also supported production of the Workplace Wellbeing Toolkit - which has an introduction by Dame Carol Black.

Outcomes

This is where I really start to struggle. For example in July 2012 Dr Susan Jebb who is chair of the food network states that ?the average amount of salt we?re eating in England has declined by .5g a day between 2008 and 2011. It shows clearly that the huge effort by the food industry to reformulate products to reduce salt is paying off?. I?ve no problem with any of that ? except that the time period and all these gains were made before the Responsibility Deal was even established!

I suspect that many companies who have a high public profile were already very involved in this agenda well before the Responsibility Deal ? for example Tesco tell us that since 2006 they have removed 3,000 tonnes of salt from their range.

It does claim to have made progress in areas such as providing information to the public through labelling ? but I am unclear how much additional progress has actually been caused by the Responsibility Deal itself.

There are 27 responsibility deal pledges in total some of the more popular ones ? particular around alcohol and food can have 80 or more signatories signing up and committing to report.

Weaker pledges where sign up and commitment to report is poor include:

Frankly some of these figures are damning. Particularly those on mental health workplace adjustments. These are measures of corporate irresponsibility ? or the irrelevance of the initiative or pledges.

Accountability

Page 6 of the launch document states that

?monitoring progress is key to establishing accountability by confirming the actions that organisations have pledged to take have been completed. Similarly evaluation has an important role to play in showing the public and other interested parties, as well as the partners themselves the impact of the Deal in changing behaviours and improving health outcomes.?

I can?t disagree with any of the above.?The problem is that the way the information is presented and the spin that is put on it means that it is not possible to meet this ambition.

The June 2012 update states that ?the Responsibility Deal has moved further and faster than could have been achieved through the regulatory route?. I can see no evidence for this. For example the update tells us that 47 partners have signed up to provide simple and consistent information for the On Trade and that together they control 19,500 premises ? sounds impressive ? however, there are almost 120,000 licensed on trade premises in England and Wales ? I am not convinced that signing up ?about a quarter of companies is moving ?further and faster.?

Unfortunately the way the data is presented ? non contextualised information; individual company information in separate spreadsheets and little aggregated data means that it is not possible to make a judgement about impact and performance

Lessons

Positives

• Some key industry bodies do seem to have bought in to the Responsibility Deal
• Many major companies do seem to be members
• Some useful tools have been produced by the industry
• There is the potential for dialogue with industry

Negatives

• Many high profile companies were already engaged in this work before the Responsibility Deal was created and the added value is unconvincing
• The number of organisations involved is very small
• The degree of buy in on important but difficult areas such as mental health at work, staff health checks and inclusion is embarrasingly poor
• It is very hard to hold the Responsibilty Deal partners and the Department of Health to account because information is presented in such a poor way and when summarised it is often ?spun? to give as positive picture as possible.
• The focus is not strategic ? with an emphasis on food and alcohol. There is a failure to engage wider sectors such as finance or housing ? to encourage them to take action to improve health
